Consultation and Report on Referred Material Recent analysis of Empire Medicare Services’ (EMS) claims data indicates that some providers are billing the Consultation and Report on Referred Material (CPT 88321, 88323, and 88325) with units of service greater than one (1). These services should be billed per case and not per specimen or per slide, and therefore, should generally be billed with a unit of service of one. Providers submitting claims for units greater than one will receive reimbursement for only one unit of service.
