Implementation of the Revised Centers for Medicare & Medicaid Services’ (CMS)-855 Medicare Enrollment Applications

Reminder: CMS has issued a revised version of the CMS-855 provider enrollment form. Providers must begin using the 04/06 version of the CMS-855 immediately. All required supporting documentation, including the NPI notification and the CMS-588, must be included at the time of filing.
