Offset by Fax

National Government Services is implementing a new process for their New York contract for providers who would like to request immediate offset of an overpayment that has already been demanded (you have received a letter requesting repayment), or a provider identified overpayment that Medicare has not yet demanded.

Requesting immediate offset is easy. Simply complete the appropriate section of the Offset Request Form and fax it to (315) 442-4282.

This form can be obtained on the National Government Services Web site at www.NGSMedicare.com. Click on Empire Medicare Services, then Part B: New York. You will find the Offset Request Form under Forms. You may also obtain the form by calling our Fax-on-Demand number, 1-866-709-1905, and requesting Catalog number 1021.
