New Address for Provider Correspondence and Internet Emails The address for Part A Provider Correspondence has changed. Effective immediately, Part A provider correspondence will be handled by the National Government Services office in Indianapolis, Indiana, and should be sent to the following address: National Government Services, Inc. Also, the Internet e-mail address for Provider Inquiries has been changed. Effective November 1, 2007, the new Internet e-mail address is ASKASFMedicarePartA@anthem.com. The Following Information is Specific to Part A Providers in New York, Connecticut and Delaware Providers should use the Internet e-mail form found in the Contact listing, under “Provider Inquiry,” on the Empire Medicare Services Web site. Go to Part A Provider Information Scroll down to the “Contacts” list and click on “Provider inquiries.” In paragraph three under “Provider Inquiries,” click on the word e-mail. This link will bring you to the page with the “Internet E-mail Form.” After reading the instructions, scroll down the page to the form.
