Retired Cardiac Rehabilitation Local Coverage Determination (LCD) September 30, 2007
National Government Services will retire the Cardiac Rehabilitation local coverage determination (LCD) (#L3863 ( New Jersey) and #L8221 ( New York)) effective September 30, 2007. For dates of service on or after October 1, 2007, please refer to our article (CMS Articles Database ID Number A45888) regarding the national coverage instructions for this service.
