Retired Local Coverage Determinations (LCDs), November 30, 2007
As part of the current consolidation of LCDs within National Government Services Medicare contracts, 00450, 00452, 00453, and 00454 will retire the Local Coverage Determinations (LCDs) listed below, effective November 30, 2007. These retired LCDs will be superseded by the consolidated LCDs listed in the last column, effective December 1, 2007.
