Revised Medical Policy Update:

The following National Government Services Policy Unit Updates for Indiana (00630) and Kentucky (00660) providers have been revised to correct a typographical error. The reported effective date for these supplemental instructions articles has been changed from July 20, 2007 to August 1, 2007. The revision histories in the supplemental instruction articles for both policies stated the correct effective date of August 1, 2007.

Source of Revision: Coding guidelines have been updated with instructions on the use of the -GA, -GZ, -GY modifiers by physicians, practitioners, or suppliers when billing claims to Medicare. References were added to the CMS National Coverage Policy section for the GA/GY/GZ modifier. Minor formatting changes made to conform to National Government Services documentation standards.
