Revised PS&R Data Request Forms and Procedures
Effective April 1, 2008, the procedure for requesting Provider Statistical & Reimbursement (PS&R) detail data has been revised. As of that date, we will no longer accept letters, e-mails, or old forms; only the newly revised form REM-512-PSR-Exh01 (02/2008) will be accepted.
Charge Structure for Detail PS&R Data
PS&R Detail Reports are available upon request. Providers may request one detail report (including sub-units) per fiscal year for cost reporting purposes at no charge. Requests for multiple years of detail reports or any other request for PS&R detail data are considered “special requests” and are outside of normal business, and thus there will be a charge. The fee for a “special request” PS&R Detail report is $200 per fiscal/cost report year.
Change of Mailing Address
The address for mailing checks with PS&R data requests has changed. Checks must no longer be mailed to your local Audit & Reimbursement PS&R Unit. The mailing address for all PS&R requests with checks is:
