Effective March 1, 2008, National Government Services added the articles listed below as attachments to the LCD for Coverage of Drugs and Biologicals for Label and Off-Label Uses (LCD ID #L25820). The articles can be found on the Centers for Medicare & Medicaid Services (CMS) Coverage Database and can be accessed on the National Government Services Web site by using the search function or by clicking on the links at the end of the local coverage determination (LCD) on the National Government Services Web site. These articles define current coverage by National Government Services. The articles will be updated in the event of changes in labeling for each drug, or if off-label use is approved by the contractor.
