Guidelines for Submitting New Jersey Inquiries

Empire Medicare Services New Jersey recently migrated to an imaging environment for inquiries. In order for us to process inquiries timely and accurately, it is important that our imaging equipment is able to read the information you send us. The following guidelines will ensure that your inquiries are processed as quickly and accurately as possible.
- Please do not highlight information. You may indicate in the margins, above or below the line item, each service in question. The service in question can also be indicated with an asterisk.
- Information that you want to bring to our attention can be noted at the top or bottom of the document.
- When underlining or annotating information, please use blue or black ink, and be sure you do not write over information.
- Please do not use gel pens. We have determined that gel pens create a distorted image.
- Please do not use correction fluid. In some instances it does not provide complete coverage. You may draw a line through the mistake using blue or black ink or use correction tape.
