2005 Medicare
Physician Fee Schedule Request
Empire Medicare Services is pleased to announce that
the 2005 Medicare Physician Fee Schedule and
Participation agreement was mailed to providers on CD-ROM
on November 15, 2004. The fees are also available on our
Web site by choosing the Fee Schedules link from the Part
B New Jersey or Part B New York index.
If you are unable to use the CD-ROM because you do not
have a CD-ROM drive, paper copies of the 2005 Fee
Schedule and disclosure materials will be available. To
receive a paper copy, fill out the request form that
is located on our Web site. The form can also be obtained
from Fax-on-Demand at 1-888-281-8586 (Document #6037) for
NJ, and 1-866-709-1905 (Document #1070) for NY.
Once you complete the form, mail your request to the
appropriate office:

Empire Medicare Services reserves the right to
apply a fee to any paper copy request. Hard copies of the
fee schedule are available at a cost of $10.55 per
copy/locality.
