Category III CPT Code® Coverage – LCD Attachment


An article entitled, “Category III CPT® Code Coverage – LCD Attachment” has been published in the Medicare Coverage Database. The article lists Category III CPT codes for which coverage will be allowed as described in the article which may be accessed in the Medicare Coverage Database effective November 22, 2007, using the article number A46075. It will also be attached to the local coverage determination, “Category III CPT Codes® Codes (L25275),” effective December 1, 2007.
