Article revised to list applicable Current Procedural Terminology (CPT)/HCPCS codes in table. (CPT codes 0024T, +0056T, and 0135T deleted effective January 1, 2008, remain in the “Abstract,” but were not listed in the table.) Clarification made to ICD-9-CM diagnosis codes that support medical necessity by adding the descriptors for ICD-9-CM codes 433.10 and 433.11 to listings with 433.30 and 433.31, respectively.
