Source of Revision – External:  Centers for Medicare & Medicaid Services (CMS) Transmittal No 77, Pub.100-02, Medicare Benefits Policy Manual, Change Request: 5680, August 29, 2007, Effective Date: January 1, 2008. List of CMS references was updated to include this transmittal. Minor changes were made to the policy to reflect National Government Services template language. The supplemental instructions article associated with this policy was also updated.

Source of Revision – External:  Centers for Medicare & Medicaid Services (CMS) Transmittal No 77, Pub.100-02, Medicare Benefits Policy Manual, Change Request: 5680, August 29, 2007, Effective Date: January 1, 2008. Coding guidelines for payable places of service were revised to remove the statement “However, ambulatory surgery centers will not receive a facility fee.” Minor changes were made to the policy to reflect National Government Services template language. The local coverage determination associated with this policy was also revised.
