Coding guidelines were revised to add place of service codes for federally qualified health clinic (50); and rural health clinic (72) when billing the technical components of CPT code 95937. References to UPIN were removed from the coding guidelines. No revision of the local coverage determination associated with this policy was required.

Please Note:  Although revision effective date is January 1, 2008, additional places of services eligible for carrier billing are effective December 1, 2007.
