HIPAA 835 V4010A1 Remittance Code
Issue
Empire Medicare Service (EMS) has been receiving a
number of inquiries regarding definitions of the Provider
Level Adjustment Codes found in the PLB segment of the
835. These definitions are documented in the 835 V4010
Implementation Guide which can be downloaded
free-of-charge from the Washington Publishing Company Web
site.
Additionally, providers should contact their
translation vendor to request these codes be translated
to text describing the adjustment; i.e., Late Charge,
Withholding, etc.
Providers are also reminded that 835 technical
questions must be directed to the IES Technical
Help Desk at 1-866-889-7322. Providers should
only be contacting the Provider Inquiry line when
questioning the payment of a claim on an electronic
remittance.
