CDS transition – Elimination of paper Medicare Part A Logon Request Form – 11/16/07

Attention All Medicare Part A FISS DDE Users: Due to the CDS transition effective 12/17/07 and our implementation of a new more efficient Medicare Part A Logon request process, we would like to inform you of the following:
- To prepare for the migration to the Companion Data Services EDC on 12/17/07, it is necessary to put a freeze on processing any new FISS/DDE requests between 11/19/07 until 12/14/07.
- NGS is taking this opportunity to roll out a new web-based process for requesting FISS/DDE access. The web form is identical to the paper form that is currently faxed to our office. This new process will be available on our web site 12 /17/07. This new automated process will resolve issues such as legibility and follow-up necessary when incomplete forms are received. Any paper forms received during the freeze will be returned to the requestor so they can be submitted through the Internet on 12/17/07. An overview of the process is as follows:
- The Authorized Signer will key and submit the FISS/DDE access request via the web form.
- The Authorized Signer must obtain a printable copy, sign, and fax the form to EDI Customer Support at: (315) 442- 4398.
- In order for your FISS/DDE access request to be processed, both the web submission and the faxed document must be received on the same day. The request cannot be processed if one is missing.
- Requests that are received and approved by 3:00 pm EST will be responded to within 7 working days from approval of the request.
- All new logon IDs and passwords will be emailed separately to the authorized Patient Account Manager that requested the information.
- 12/17/07 the new form will be completed on the web site at: http://www.empiremedicare.com/omnipro/forms/mc1064.htm
