Ibandronate Sodium (e.g., Boniva®) – Related to LCD L25820 (A46087)
Article published May 2008: Source of revision – internal: The article text paragraph has been revised for clarity and to correct the spelling of pharmacopoeia. The headings under the “Coding Guideline” section have been revised. The name of the drug has been corrected in the place of service guidelines for carriers. Bill types have been added.  

Intravenous Immune Globulin (IVIG) - Related to LCD L25820 (A47381)
Article published May 2008: Source of revision – internal: removed multiple sclerosis from the “Limitations” section of the article.
