Correction - Coverage for Lucentis (ranibizumab injection)

In the Medicare Information Resource, MIR-2006-8A, August 2006, instructions in the second paragraph were incorrect. Providers were advised to use the HCPCS code C9224 for dates of service on or after October 3, 2006. Please do not use the HCPCS code C9224 for this drug. Only the HCPCS code C9399 should be used for this drug until a HCPCS code is assigned by the Centers for Medicare and Medicaid Services (CMS).
