PS&R Data Request Payments
Certain requests for Provider Statistical & Reimbursement (PS&R) data require prepayment. Any checks for PS&R data requests should continue to be mailed to the address on the data request form (MC4383). PS&R checks should not be mailed the P.O. Box in Newark, NJ.
