- Note: The fee schedules posted here are the revised 2006 fee schedules that became available on February 9, 2006 as a result of the Deficit Reduction Act.
- Participating Physician/Supplier Agreement offers providers the opportunity to be either a participating or non-participating provider in the Medicare program. Instructions on how to fill out and submit the agreement form are included. Participation Agreements for 2006 must be postmarked on/before March 31, 2006. However, the effective date of the Participation Agreement is retroactive to January 1, 2006.
- CMS Medicare Physician Fee Schedule Page:
 Provides more than 10,000 physician services, the associated relative value units, a fee schedule status indicator, and various payment policy indicators needed for payment adjustment.
