• Participating Physician/Supplier Agreement offers providers the opportunity to be either a participating or non-participating provider in the Medicare program. Instructions on how to fill out and submit the agreement form are included. Participation Agreements for 2008 must be postmarked on/before December 31, 2007. However, the effective date of the Participation Agreement is January 1, 2008.
• CMS Medicare Physician Fee Schedule Page: Provides more than 10,000 physician services, the associated relative value units, a fee schedule status indicator, and various payment policy indicators needed for payment adjustment.
